Dental requirments for students entering K, 2nd, 6th, and 9th grades.
- Dental exams are required for these grade levels.
- This serves as a reminder that all students in grades Kindergarten, 2nd, 6th and 9th grades will need to submit a dental exam by May 15th of the current school year.
- Reminders will be sent out in the winter of the current school year to all students who still need a dental exam submitted to their school.

Illinois School Physicals are required to be on file in the school's health office on or before October 15th of the current school year or date set sooner by the school district.
The following grade levels will need to have these completed documents completed: New PRESCHOOL students, KINDERGARTEN, SIXTH and NINTH grades as well as students transferring in from OUT of STATE.
Students in grades 6th and 9th will not need a seperate Sport/Activity physical as those grades are mandated to have a School Physical.
Sport/Activity physicals are a separate physical form needed for students who will be involved in extra-curricular activities. Grade levels 6th -12th will need a Sport's/Participation physical completed prior to tryouts.
If your student has a completed up-to-date Illinois School Physical, that form will qualify for a Sport/Activity Physical.
School Physicals are valid for 365 days, Sport/Activity Physicals are valid for 395 days.
Please be advised that October 15th of the current school year or date set sooner by the school district is the cut-off date to have SCHOOL PHYSICALS and updated VACCINATION records turned into the Health Offices to avoid exclusion from school.

Children entering Preschool have 30 days to turn in completed school physicals and immunization records. The 30 day starts on the day of registration.

ELEMENTARY SCHOOL - 99.7% COMPLIANT WITH IMMUNIZATIONS AND PHYSICALS
MIDDLE SCHOOL - 100% COMPLIANT WITH IMMUNIZATIONS AND PHYSICALS
HIGH SCHOOL - 99.5% COMPLIANT WITH IMMUNIZATIONS AND PHYSICALS
IMMUNIZATION WAIVERS - 3.2% RELIGIOUS AND MEDICAL EXEMPTIONS
Vision and Hearing screenings will commence during September and continue through the school year for students in the following grades:
Vision Screenings mandated per IDPH and ISBE: Pre-K, K, 2, 8 and all children with IEP's in the district.
Kindergarten students must have a vision exam by an Optometrist or Opthamologist completed by October 15th each year.
Students transferring into Illinois schools must have a vision exam by an Optometrist or Opthamologist completed 90 days after transferring.
Hearing Screenings mandate per IDPH and ISBE: Pre-K, K, 1, 2, 3 and all children with IEP's in the district
If the parent/guardian does not want their student screened please put in writing and send to your student's nurse. (This serves as notice per IDPH request.)
Vision exams are required for students enrolling in Kindergaten and any student enrolling in an Illinois school for the first time.
School vision exams are required to be completed by a licensed optometrist or medical doctor who preforms vision exams as specified in IDPH.
Vision exams are required to be on file on or before October 15 of the current school year.
