Physical Information
The State of Illinois Certificate of Child's Health Examination form is due October 15th of the current school year. (Because October 15th is a Sunday, all physicals will be due by 10/13/2023)
Please make appointments soon to have School Physicals and Sport/Activity Physicals completed. Numerous Urgent Care, Med Care and other walk-in clinics are available for school physicals as well.
Dental Information
The State of Illinois Dental Examination is due on or before May 15 for grades K, 2, 6 and 9 of the current school year. (5/15/2024)
Completed forms may be dropped off at the schools or faxed to the school office at (815) 802-7484.
- Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent.
- Pre-Schooler's must have at least one dose of the pneumococcal vaccine as well as completed vaccine record
- Physicals are due within 30 days of starting Pre-School.
KINDERGARTEN
- Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent.
- Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year.
- Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline. (EXAMS DATED BETWEEN 11/15/22-5/15/24)
SECOND GRADE
- Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the current school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
SIXTH GRADE
- Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent.
- Proof of one dose of the Tdap vaccine for students entering sixth grade.
- Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the current school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
- Proof of one dose of MCV4 vaccine on or after the 11th birthday.
NINTH GRADE
- Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent. The Illinois School Exam Form for Physicals is also accepted as an athletic physical.
- Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the current school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
GRADE TWELVE
- Proof of MCV4 vaccine, 2nd dose if needed
NEW STUDENTS FROM OUT OF STATE
- Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history
- Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school or a new vision exam from an Illinois optometrist/ophthalmologist is expected by October 15th of that school year.
STUDENTS TRANSFERRING FROM OTHER ILLINOIS SCHOOLS
- Transfer of current Illinois school health records (This includes: physical exam with up-to-date immunizations, dental exam and eye exam.)

MEDICAL OBJECTION
- A statement from the MD, DO, APN or PA indicating that an immunization is "medically contraindicated" is acceptable and will be attached to the student's physical exam form in lieu of the vaccination. In case of disease outbreak, the student must be excluded from school for 14 to 21 days of school, mandated by IDPH.
- Should the condition of the child later permit immunization, the requirement will then have to be met.
RELIGIOUS EXEMPTION
- Requires the State of Illinois Form with a physician signed statement explaining the objections to physical exam, health screenings, and/or immunizations on religious grounds. General philosophical or moral reluctance will not provide a sufficient basis for exemption. In case of disease outbreak, students not up to date on immunizations must be excluded from school for 14 to 21 days of school, mandated by IDPH.
PERSONAL IMMUNIZATION EXEMPTIONS ARE NOT VALID IN ILLINOIS PUBLIC SCHOOLS
Sport Participation and Activity Physicals
- A yearly Sports Physical is required for all Athletes to participate on school sponsored extra-curricular athletic/activity programs.
- The 6th and 9th graders will not need a separate sport physicals
- All athletes in grades 7, 8, 10, 11 and 12 are required to have a yearly sport physical to participate in any school sponsored extra-curricular athletic/activity program.
- Sports Physicals are good for 395 days from the date of the physical.
