*Students in the Early Childhood Center and Elementary  Schools will not be administered cough drops/lozenges
due to choking hazards.

OVER THE COUNTER MEDICATION (OTC)

Cough And Vector Image Illustration Of Syrup - Child Medication ...

Any over- the- counter medication (OTC'S)  such as cough drops, antacids, headache remedies,
and/or other OTC medications, will only be administered  at school, during school hours, when accompanied by the:


Parent Request for Administration of  OTC Medication at School form
or a form generated through the prescribing physicians offices.


All OTC medications must be in the original packaging, with the students name, name of medication
and dosage, as well as the diagnosis for administering the OTC medication.

PRESCRIPTION MEDICATION INCLUDING SELF-ADMINISTERED MEDICATIONS
(INHALERS, DIABETES MEDICATIONS, EPI-PENS, etc.)

Any prescription medications will only be administered when accompanied by the:

1.  Parent Request for Administration of  Prescribed Medication at School form
or a form generated through the prescribing physicians offices. 

2.  Prescription medications must arrive in the pharmacy containers with all identifying information noted.  

3.  Prescription medications will be administered at school during school hours when ordered by the prescribing physician.

***School Nurses have the confidential discretion to contact prescribing physicians if questions arise regarding medication administration to students.***


*Students will not be administered a prescription medication and/or an OTC medication
without the completed Medication Authorization form which has been signed by the parent/guardian and the health care provider.
